PATHOPHYSIOLOGY OF ITCHING

Due to the causative factors irritation in the skin occurs. The feeling of itching on the skin arises due to stimulation of pruriceptors by mechanical, thermal or chemical mediators. Histamine involved in the pathogenesis of pruritoceptive itch.

DIAGNOSIS OF ITCHING

  • Itching is diagnosed by the proper inspection of the site.
  • Laboratory tests to find out the underlying pathology

TREATMENTS FOR ITCHING

  • Topical steroids
  • Phototherapy
  • Antihistamines
  • Antibiotics – itching due to bacterial infection

PROGNOSIS OF ITCHING

  • The prognosis is based on the pathophysiology of itching.

COMPLICATIONS OF ITCHING

  • Scratching causes damage to the skin
  • Sleep disturbances
  • Mental irritations

ITCHING AND AYURVEDA

  • In Ayurveda, itching is called ‘kandu’. It is the symptom of many skin diseases like vicharchika, kitibha, vipadika, dadru etc.

 

NIDANA- AYURVEDIC CAUSES OF ITCHING

  • Overexposure to allergens (agantu factors)
  • Over vitiation of kapha dosha
  • Virudhahara (incompatible food)

 

PURVAROOPAM- AYURVEDIC PREMONITORY SYMPTOMS OF ITCHING

  • Irritation in the skin with or without an external factor

 

SAMPRAPTI – AYURVEDIC PATHOGENESIS OF ITCHING

Due to the causative factors, dosha becomes vitiated predominantly kapha and pitta and further vitiates the rakta dhatu . This causes an uncomfortable sensation in the skin in the form of itching.

LAKSHANA- AYURVEDIC SIGNS AND SYMPTOMS OF ITCHING

  • Itchy lesions on the skin– kandu
  • Daha – burning sensation
  • Sopha – inflammation
  • Rakta varna – reddish colour

AYURVEDIC PROGNOSIS OF ITCHING

  • Prognosis is Sadhya –curable with appropriate ayurvedic management

CHIKITSA- AYURVEDIC TREATMENT FOR ITCHING

Ayurvedic local treatment for itching

 

  • Lepa – with cooling drugs like a paste of chandana, ramacha, lodhradi churna on the affected area (not much thick, not during at night, avoid drying)
  • Jatyadi taila
  • Nalpamaradi tailam
  • Vilwadi gulika for external application

 

AYURVEDIC SAMANA TREATMENT FOR ITCHING

  • Guduchyadi kashaya
  • Avipatti churna
  • Punarnavadi kashayam
  • Sarivasava – after food
  • Patoladi kashayam
  • Aragvadhadi kashayam
  • Vilwadi gulika
  • Haridrakhanta – along with milk

AYURVEDIC SHODHANA TREATMENT FOR ITCHING

  • Virechana – purgation

HOME REMEDIES FOR ITCHING

  • Wear a loose dress in the hot climate
  • Intake of gooseberry – cools the body
  • Moisturizing cream application

DIET AND BEHAVIOUR FOR ITCHING

  • Avoid food items which produce dosha imbalance like cold, acrid, sour and dry food, hard to digest. Because dosha imbalance aggravates the symptoms.
  • Intake of proper amounts of fluids
  • Avoid more intake of black gram, jaggery, fish
  • In the case of infections, towels are not shared with others

YOGA FOR ITCHING

  • Nadi shuddi pranayam – calms the mind and cools the body

 

The patient needs to be seated in a meditative posture with the head and spine erect, with the body relaxed. The patient has to close his one nostril (e.g. left nostril if using the right hand and vice versa) with the thumb and exhale completely through the other nostril. Again, he will have to breathe in deeply through the other nostril while the opposite nostril is still closed with the thumb.

  • Bhastrika pranayama
  • Sheetali pranayama
